Introduction
T2Z Care Services was established in 2019 and since then we have supported many individuals, from living in care support to pailliative care, domiciliary care and supported living services.

Our services commenced with our first customer just 4 days into the first lockdown of 2020, immediately we knew we had to support with the demand and the pressures that Covid-19 had inflicted upon health and social care.

We partnered with a local domiciliary care provider in support of Gloucestershire County Council's 'Winter Capacity Alliance' and branched out to support in Gloucester and Forest of Dean.

During these first 2 years, we knew it was essential to take a creative and modern approach to providing care and support, with a strong focus on digitalisation and technology.

As of October 2022 we have also been providing supported living services to adults with learning difficulties and disabilities, autistic spectrum disorder and mental health.

As we continue to support vulnerable adults throughout Gloucestershire, we inherit a strong and positive attitude toward continuous investment in improvements to our service delivery and our reputation as a preferred provider of choice.

T2Z has always approached care from a modern perspective, consistently thinking on the lines of continuity and longevity.

We have a vision for the future and intend to launch our services on a scale to help support even more people as we continue to establish ourselves as a preferred provider of choice.

As well as ensuring that training provided meets compliant standards, it's also important that our workforce are trained in need specific aspects and with our mentorship programme we ensure support is provided to ensure continued learning and development.

It is imperitive that we sustain the ability to see potential in people and utilise their strengths and giving them the opportunity to shine with a true leadership approach that enables T2Z to hold a culture and operate in a way that you don't often see within the sector.

T2Z has always been an outcomes based provider, a strong ability to recognise what is going right or what is not? And responding to this with the implementation of changes that in most cases improve services and quality of lives of the people we support.

Our supported living services are very much 'homely' homes and it is important that support staff as well as the people we support work and live in an environment that is inviting, safe and comfortable.

It helps them to feel that their skills and abilities are valued. It creates a healthier work and home environment, with better outcomes and retention. It also creates a sense of belonging among the team that can translate to greater productivity, more innovation, and better decision-making. Team performance improves when individuals feel more connected and having that approach at T2Z allows us all to celebrate who we are as individuals as well as a team.

TRANSITION PLANNING

Transition plans are an important aspect of any planning taking place prior to an individual moving into our services. This is extremely important for those with any anxieties and/ or behaviours that could impact other individuals they might be living with and/ or local communities. Getting the plan right is key to a healthy transition and prevention of early placement breakdowns due to careless planning. In a situation where an urgent case might arise and an individual requires accommodation and support rapid - we are skilled in creating strategies to support 'safe' and 'comfortable' transitions into our service, at pace. A mindful approach to transitions with use of a client compatibility assessment along with client vs carer matching is also key.

PERSON CENTRED CARE

Person-centred care supports people to develop the knowledge, skills and confidence they need to more effectively manage and make informed decisions about their own health and care. It is coordinated and tailored to the needs of the individual, and has been proven to improve both physical and mental wellbeing. As a team, we have a person-centred approach to our work; which involves calmly and practically responding to those we support to achieve the best outcomes. We treat people with dignity, compassion, and respect. Here at T2Z, we pride ourselves on putting those we support at the heart of our care, which is demonstrated by the service we deliver.

RESEARCH & DEVELOPMENT

We have always been inspired by a modern approach to care and support. With the use of digitalisation and technology, following the route of NHSx and a firm focus on this to improve quality and compliance as well as improve peoples quality of life. T2Z are a digital driven care provider and we are known for being creative in our approach. If done correctly, you can better record and evidence with the use of electronic reporting, as well as become more effective and responsive. The use of electronic reporting can reduce risks and monitor progress and trends to give us a database to support improvements and service delivery.

BREAKDOWN PREVENTION

Placement stability is an important element of permanence as it creates contingency and opportunities for those we support, to develop good working relationships and for them to feel safe within the environment they live. Having a range of options and identifying the right placement for each individual is key to stability and permanence. Identifying the right placement requires effective assessment and planning processes and listening to the views and wishes of the individual and their loved ones. Stability for those we support is key.

POSITIVE BEHAVIOUR SUPPORT

T2Z Care Services are historically skilled in behaviour management. PBS stands for 'positive behaviour support'. With creative use of distraction and diffusion techniques along with person centred processes, ensuring medication and restrictive interventions are a very last resort. We believe that consistency in approach and script, along with thorough support planning and risk assessments are the key to reducing behaviours to benefit the people we support to develop an increase in well-being and a quality of life. It's at this point it is clearly visible the benefits of the use of PBS.

Live-In Care
Our live-in care model is offered based on our previous experience of providing domicilliary care services and previous live-in support, and is now developed in line with our skills and values and established experience of providing supported living services. 

We develop each, individual live-in care package with a tailored and person centred approach, ensuring that we fairly assess each individuals needs, which in turn determines fair pricing.

As you might guess, each individuals needs vary, this then reflects the amount of skillset required for the specific role, the intensity of the tasks required and the necessary 'training specific' investment to ensure the worker is prepared to meet specific needs of the individual or individuals they might support.
If we aim for outstanding, but achieve good - then I guess that's great.

We are registered and regulated by the CQC  

The Care Quality Commission (CQC) use the evidence they collect to reach judgements about the quality of care. They publish reports about the services they inspect on their website. After each inspection, they produce a report. In most cases their reports include ratings, which show their overall judgement of the quality of care. CQC's reports set out what their findings on each of the five key questions mean for the people who use the service. CQC describe the good practice they find, as well as any concerns they have. They clearly set out any evidence about breaches of regulations. They also make recommendations to help the care provider improve their rating.

Quality control:
CQC aim to be consistent in everything they do. To help them achieve this they set up quality assurance panels to look at samples of their rating judgements to check consistency.

Action planning:
We, the care provider must respond to areas of concern that CQC have identified, develop an action plan to address them and make improvements. CQC will follow up on any actions they tell care providers to take. They may follow up by contacting the care provider or visiting the service to carry out a focused inspection.

When the CQC inspect acute hospital trusts, specialist mental health services and community health services, their inspection findings are discussed at a quality summit. This is a meeting with the care provider and partners in the local health and social care system.
