Transition plans are an important aspect of any planning taking place prior to an individual moving into our services. This is extremely important for those with any anxieties and/ or behaviours that could impact other individuals they might be living with and/ or local communities. Getting the plan right is key to a healthy transition and prevention of early placement breakdowns due to careless planning. In a situation where an urgent case might arise and an individual requires accommodation and support rapid - we are skilled in creating strategies to support 'safe' and 'comfortable' transitions into our service, at pace. A mindful approach to transitions with use of a client compatibility assessment along with client vs carer matching is also key.
Person-centred care supports people to develop the knowledge, skills and confidence they need to more effectively manage and make informed decisions about their own health and care. It is coordinated and tailored to the needs of the individual, and has been proven to improve both physical and mental wellbeing. As a team, we have a person-centred approach to our work; which involves calmly and practically responding to those we support to achieve the best outcomes. With the use of digitalisation and technology, following the route of NHSx and a firm focus on this to improve quality and compliance as well as improve peoples quality of life. T2Z are a digital driven care provider and we are known for being creative in our approach. If done correctly, you can better record and evidence with the use of electronic reporting, as well as become more effective and responsive. The use of electronic reporting can reduce risks and monitor progress and trends to give us a database to support improvements and service delivery.
Placement stability is an important element of permanence as it creates contingency and opportunities for those we support, to develop good working relationships and for them to feel safe within the environment they live. Having a range of options and identifying the right placement for each individual is key to stability and permanence. Identifying the right placement requires effective assessment and planning processes and listening to the views and wishes of the individual and their loved ones. Stability for those we support is key.
T2Z Care Services are historically skilled in behaviour management. PBS stands for 'positive behaviour support'. With creative use of distraction and diffusion techniques along with person centred processes, ensuring medication and restrictive interventions are a very last resort. We believe that consistency in approach and script, along with thorough support planning and risk assessments are the key to reducing behaviours to benefit the people we support to develop an increase in well-being and a quality of life. It's at this point it is clearly visible the benefits of the use of PBS.
